By registering a student for a class, you agree to the following policies:
Photo Release
I, the undersigned, hereby give Dance Extreme, its agents, and/or assignees permission to use the photographs, motion pictures, or any reproductions of the above student’s physical likeness taken of me in any manner it deems proper. I relinquish all rights, title, and interest I may have in the finished pictures, negatives, and copies. I waive the right of prior approval to any finished products, advertising copy, or other matter or use that may be applied, and hereby release Dance Extreme, its agents, and /or assignees from any and all claims from damages of any and all kinds based on the use of said material. I hereby warrant that I am a parent or legal guardian of the subject of photography, who is under eighteen years of age, and am competent to act on his/her behalf insofar as the above is concerned.
Liability Release
I do hereby agree to release Dance Extreme and all other cooperating agencies, employees, officials, or managers thereof, from all liability for damages by reason of injuries or property damages that may be sustained as a result of participation in this program.
Medical Waiver
I authorize all medical and surgical treatment, X-ray, laboratory, anesthesia, and other medical and/or hospital procedures as may be performed or prescribed by the attending physician and/or paramedics for my child and waive my right to informed consent of treatment. This waiver applies only in the event neither parent/guardian can be reached in the case of an emergency.
Medical Consent
In the event of injury, I hereby authorize the program officials of Dance Extreme to arrange for medical services as may be deemed reasonable and necessary to the welfare of the injured, and I do hereby release Dance Extreme and all others from all liability in taking action, including all action which may be contrary to personal religious beliefs. I, the undersigned, have read this release and consent to medical treatment and understand all its terms. I execute it voluntarily and with full knowledge of its significance.
Tardies and Absences
Please be on time for class to ensure proper warm-up and not disrupt the other dancers. If you are late, please warm up on your own on the side before joining class. Our classes are set up based on the season, and because of this, we don’t offer tuition adjustments or make-up classes for absences. The monthly tuition remains the same, regardless if there are three, four, or five classes within the month. Classes are NOT pro-rated.
Attendance
Students are expected to attend class every week to ensure consistent learning and advancement. Students must attend class before any performance in order to participate in the following performance event. If too many classes are missed and the student is not proficient in the routine, the student will not perform with the class.
Performance Fees
Some performance events may require an entrance fee, admission fee, and/or recital fee.
Costume Fees
The costume fee is due Oct. 1st. Fees not paid by that date will be included with the automatic tuition. Costume fees range from $55-100 plus cost of shoes and tights for each dance routine. Costumes will not be ordered until paid. If you are late, you risk having to pay separate shipping fees,and not getting your costume in time for a performance. Students will not be allowed to perform without the costume and shoes required.
Dress Code
The dress code will be enforced. A student will be sent home if the dress code is not followed. See dress code under the Classes tab.
Class Rules
Any disruptive students will be asked to leave the class. No gum, food or drinks in the studio. Only water botttles will be allowed. No cell phones in class. All students must wait inside the building for parents to arrive- please be on time.
Parents
Parents are not allowed in classrooms during class time. Please do not interrupt class to talk to the teacher. Please do not coach from the sidelines. If you have questions or concerns, please email the director. Please be prompt in picking up your children.
Communication
We commit to making every effort to communicate with parents including written notices sent home, email, text messages, social media, and the website. Please subscribe to our Facebook and Instagram pages for announcements. Please check your email often for notices and reminders. Please keep your most current contact information on file. Behavior
We reserve the right to dismiss, suspend, or expel any person who is disruptive and/or violates studio policies. Any disrespectful and/or disruptive students and/or parents will be asked to leave the program as the instructor/owner/director sees fit.
Studio Closures
We reserve the right to close the studio and cancel classes in the event of teacher illness or inclement weather. We will make every effort to find a substitute for teacher illness. If the Weiser School District is closed due to inclement weather, the studio will also be closed. We will email parents and post to social media if a closure is necessary. No refunds will be issued.
Dropping Classes
If a student needs to change a class or must drop a class, please notify the studio by email. Tuition will be charged at the regular rate until the notice is received. To stop your automatic payment for the next month, the director must receive your notice by the 1st of the month. Costume fees are non-refundable once the order has been placed.
